Sorry I don’t have pics, but yes it looks just like that one. It is in the wood box(says Tube Maze on the screen on the box as well) and has a silkscreen on the side of the pedal also.

No, they (3 models?) all have pre-loaded samples (6 or so?) and you can choose 1 of those with a rotary switch (IIRC), one of the more interesting ones is “breaking glass”
Should work if you don’t have too much gear on that power strip: have a look at the Amperes of the converter
I use a small one for a few old US things like my vintage EHX Vocoder and I know there was a much heavier model for more gear that needs more power.
I’m in Belgium, our plugs work in a lot of European countries, but not the UK (flat pins) and sometimes the earth is different too. You can buy travel adapters for that (just make sure your converter fits on the other side )
